Pathways™ is an educational and supportive program for people facing an advanced or chronic illness. The Pathways™ program is a community outreach service of Hospice of Wichita Falls and is offered free of charge to participants as well as their family members and/or their caregivers. Pathways™ covers topics that can include but are not limited to:
- Communication concerns
- Defining quality of life
- Coping strategies
- Practical issues
- Emotional concerns
- Spiritual concerns
- Advance Directives
Pathways™ visits can be provided in the comfort of the home, the hospital, or another agreed upon location and the number of visits will be determined by the patient, the family, and the Pathways™ facilitator. At the introductory meeting, the program is personalized so that it best fits the desires and the needs of the patient and the family. The facilitator offers support, education, encouragement, advocacy, and referrals to outside resources if necessary. It is important to know that the program does not involve medical care or medical advice, which continues to be directed by the patient’s physician.
The goals of the program are:
- To assist patients and families to understand choices and determine directions as they pursue treatment
- To be life-enhancing through education, support, and decision-making
